Notice on the Collection, Handling, and Use of Personal Information Collected from the Shin Kong Financial Holding Co., Ltd. "Feedback Form"
In accordance with provisions in item 1, Article 8 of the Personal Information Protection Act (PIPA), Article 16 of the Enforcement Rules of the Personal Information Protection Act, and the reference template for notification content in the Bankers Association of the Republic of China letter Quan-Fa-Zi 1011000896A, dated September 27, 2012, the following notification is made regarding personal information collected from the "Feedback Form." Please read carefully: 1. Purpose of collection:
	Personal information is used for specific purposes within the scope of consumer and customer management and services and other advisory and consulting services.
2. Types of personal information collected (detailed in the Company's personnel forms and contracts):
	(1) Identification (Including: Name, e-mail address, mobile phone, and home phone) (2) Characteristics (Including: Gender and nationality) (3) Employment status (Including: Company and title) (4) Financial details (Including: Investments)
3. Time period, area, target and method of the use of personal information:
	(1) Time period: The Company may retain personal information for up to one year after responding to a suggestion or question; or for the duration required for the execution of business. (2) Area: Taiwan (3) Target: The Company, agencies with legal investigative powers, and financial supervisory agencies (4) Method: Usage by automatic or non-automatic methods
4. Under Article 3 of PIPA, you may exercise the following rights in regards to your personal information retained by the Company:
	(1) You may submit an inquiry or request to the Company to review or make duplications of personal information. The Company may, according to the law, charge a due fee for such services. (2) You may request the Company to supplement or correct personal information. However, the law requires that you provide an appropriate explanation. (3) You may request that the Company discontinue collection, processing or use of personal information and request that such information be deleted. However, the Company may, according to law, refuse to comply with such requests when the information involved is necessary for the execution of business.
5. How your rights and interests may be affected by not providing personal information:
	You have the freedom to not provide related personal information. However, if you refuse to provide such information, the Company may be unable to provide related customer management or other consulting services.
